SLG Benefits and Insurance, LLC markets and underwrites group life insurance products on behalf of UnumProvident (press release).  A full range of basic Employer Life and AD&D products along with Voluntary Life and AD&D products are available for the select business partners of SLG Benefits.  UnumProvident represents a leading fully insured group product portfolio that now – through this exclusive marketing / underwriting agreement with SLG Benefits – are available to employer groups that work with TPA’s.

All UNUM group life insurance programs secured through SLG Benefits will include the following as Standard Plan Design: Survivor Support, Portability, Travel Assistance (Assist America) and the Work Life Balance Program (EAP).  Additionally the Standard Plan Design will include (but not limited to) the following AD&D features:  Seat Belt / Airbag, Repatriation, Education and Exposure & Disappearance.  All plans will also include a minimum Two Year Rate Guarantee.

PORTABILITY

Portability is the ability for a covered employee to continue group life coverage – at group rates – when the coverage would otherwise end.

Conversion is the ability for a covered employee to convert their group life coverage to an individual coverage without providing evidence of insurability.

SURVIVOR SUPPORT

Survivor financial counseling services

These services, provided by The Ayco Company L.P.1 provide objective financial planning and advice to survivors after the death of an employee or insured spouse or to terminally ill employees or their insured spouse.

  • Expert, objective financial advice at no additional charge from highly trained attorneys

  • Development of a detailed, personalized financial plan

  • Guidance and counseling for up to 12 months, through toll-free telephone access

At no point will an advisor offer or sell any product or service. Financial counseling services bring welcome support at a time when it’s needed the most. A specially trained attorney will initiate phone conversations with the beneficiary to develop a personalized plan based on the individual’s circumstances — regardless of the life benefit amount.

Worldwide emergency travel assistance services

Just one phone call gives employees and their families 24-hour access to a network of emergency medical and legal resources any time they travel more than 100 miles from home or in a foreign country.

UnumProvident’s worldwide emergency travel assistance services, provided through Assist America, Inc., one of the nation's largest providers of emergency travel programs, provides travelers with a range of features, including pre-departure information and assistance finding lost luggage.  More importantly, it is there for medical emergencies, providing:

  • Access to English-speaking and Western-trained doctors or facilities anywhere in the world or evacuation to where such care is available
  • Repatriation under medical supervision when necessary

Other available services include 24-hour multi-lingual service, medical consultation, hospital admission guarantee, critical care monitoring, emergency prescription services, return of mortal remains, care for minor children and legal and interpreter referrals.

 WORK-LIFE BALANCE

Work-life balance employee assistance program is a comprehensive workplace effectiveness program that provides information to assist employees and their families with managing general life issues and stresses ranging from everyday to severe. The work-life balance program provides 24-hour access to counseling and information to help reduce absenteeism and enhance productivity, including referrals to third-party providers for face-to-face counseling when needed.

Retained Asset Security Account

When a beneficiary makes a claim that is $10,000 or greater, Life Benefits establishes a Money Market Checking Account at State Street Bank of Boston in the name of the Beneficiary.  State Street Bank of Boston sends to the Beneficiary:

  • A checkbook, and
  • Account information

The Beneficiary then chooses to:

  • Keep the funds in the account indefinitely
  • Close the account immediately by writing a check for the balance, or
  • Write the checks as needed on the insurance proceeds

Accelerated Benefit

The standard accelerated benefit is 50% (up to the plan maximum, but not to exceed $750,000).

Other accelerated Benefit Options are:

  • 75% (up to the plan maximum but not to exceed $500,000)

  • 100% (up to the plan maximum but not to exceed $250,000)

 

Standard AD&D Benefits

Education Benefit

The education Benefit provides up to $24,000 (6% to $6,000/year) for the financial support of surviving, unmarried children under the age of 25 of an insured employee who dies as a result of accidental bodily injury within 365 days of the accident and who are enrolled in an accredited post-secondary institution of higher learning.  Up to 4 benefit payments will be paid to the authorized representative of each qualified child who is:

  • Enrolled full-time in an accredited post-secondary institution beyond 12th grade, or

  • Currently at 12th grade level, but who enrolls full-time in an accredited post-secondary institution beyond 12th grade within 365 days of the death of insured

The maximum benefit period is 6 years from the date the first benefit has been made.

 

Exposure and Disappearance

The Exposure Benefit provides coverage if the insured (employee or dependent) is injured and unavoidably exposed to the elements and as a result of the exposure, suffers a covered loss.  The benefit is paid according to the standard loss schedule. 

The Disappearance Benefit presumes the insured to suffer loss of life due to an accident if he/she is:

  • Riding in a common carrier that is involved in an accident; and

  • the common carrier is wrecked, sinks, is stranded, or disappears; and

  • the insured's body is not found within 1 year

The full benefit is paid for loss of life.

 

Seatbelt/Airbag

The Seatbelt Benefit provides an additional 10% to $25,000 if an insured dies in a car accident while wearing a seatbelt that was properly fastened.  If it is unclear if the insured was wearing a seatbelt, UnumProvident will pay $1,000.  The Airbag Benefit provides an extra 5% to $5,000 if the insured was protected by an air bag in addition to the seat belt (total combined Seatbelt/Airbag Benefit up to $30,000).  The deceased must have been protected by the airbag for the benefit to be payable.  For example, if an insured were a passenger in a car that had a driver's seat airbag only, the benefit would not be payable.  The benefit will not be paid if the airbag is disengaged.

 

Repatriation Benefit

The Repatriation Benefit provides an additional benefit of up to $5,000 to pay for the preparation and transportation of the deceased to a chosen mortuary, when death is the result of an accident which occurs more than 100 miles from the insured’s primary residence.  Repatriation benefits are paid following payment of the AD&D benefit.
