Preparing Claim Documentation

When the TPA submits claims for reimbursement on cases involving third party liability, we must receive the following important documentation:

1. SLG Liability Questionnaire Form (or similar TPA form) completed by either the TPA or the Policyholder. Please include any appropriate attachments.

2. A Subrogation and Right of Recovery Reimbursement Agreement signed by the Policyholder.
